The University of Arizona Press (hereafter the Press) is pleased to have the privilege of making use of your photographs in accordance with the terms of the following agreement:

1. The Press shall have the right to use your photograph(s), consisting ___________________________________________________________________________________________________________________________________________________________________

as part of the volume now titled ______________________________________ by _____________________________ and in subsequent printings and editions of that volume and in related advertising.

2. You grant the Press the nonexclusive right to reproduce the photograph(s) in (title)_____________________________ to be published by the Press in the English language in the United States of America and Canada and their territories and possessions.
3. The Press shall have the right to reprint the photographs in connection with any reprinting, revision, or reproduction of _______________________________________ in any language and/or medium, whether now known or hereafter devised.
4. You will retain ownership and copyright of your work and all other rights not specified herein.
5. This agreement is executed as of the date of signature, below.
6. You represent and warrant that you are the sole author and owner of all rights in and to the photographs and that that the photographs are original and do not infringe upon any statutory copyright or any common law right or proprietary right of any individual or organization.

Please confirm that the foregoing accurately and completely sets forth our agreement by signing and returning the copy hereof.
